アニメ「劇場版モノノ怪」シリーズ三部作の第三章。退魔の剣を携えた薬売りと唐傘による死闘、その後の火鼠との決戦を経た大奥に、ようやく平穏が訪れたように見えた。しかし薬売りはなにかの気配を感知し、警戒を続けることに。そんな折、天子の正室である幸子が待望の男児を授かるも、ほどなくして赤子は亡くなってしまった。世継ぎを産み、天子との形だけの夫婦関係を変えたいと望んでいた幸子の無念と怒りは、しだいに怨念へと変わり大奥内で不自然な地震や怪事件が続発。駆けつけた薬屋の前に現れたのは大蛇の姿をしたモノノ怪、蛇神だった。
